Care Coordinator
Wolverhampton (office-based with field visits) Full-time £28,000 – £32,000 per annum
About the role
Coordinate carer rotas, build care plans with clients and families, and act as the single point of contact for ongoing care delivery.
Key responsibilities
- Build, monitor and adjust client care plans
- Coordinate weekly carer rotas and cover for absences
- Conduct initial client assessments and reviews
- Maintain compliance with CQC standards and internal policies
What we're looking for
- NVQ Level 3+ in Health and Social Care
- Previous coordinator or scheduler experience preferred
- Strong organisational and IT skills
- Full UK driving licence and access to own vehicle
